Visa Waiver/Change of Status Application
Before submitting your application through mail or drop off, you must first submit your details Here and continue below for the application. We will return back your application if we do not receive your details.

Documents required for Visa Waiver Certificate:
- Copies of form IAP- 66.
- Passport.
- U.S. visa and a copy of job termination letter/notice (if applicable).
- Money Order or Cashier Cheque: Money order or Cashier Cheque of US $25/- for each applicant (with at least one year’s validity) payable to the Embassy of Nepal, with the name and address of the applicant written on it.
- Two Pre-paid Return Envelopes: Submit prepaid return envelopes of 9” x 12” size with tracking number and address of the applicant to send the Documents back to you and State Department once it get ready. (Prepaid return envelope is not required for self-pickup).
Note: Submitted application will be returned back immediately, if we do not receive a complete documents mentioned above.
